• Provide assessment, advocacy, and discharge planning for the psycho-social needs of patients of all ages.
• Demonstrate expertise in crisis intervention, mandated reporting, and care coordination of patients with psychosocial risk.
• Prioritize multiple tasks and possess strong interpersonal skills.
• Exhibit excellent communication and maintain a professional demeanor with patients, families, interdisciplinary team members, and outside agencies.
• Coordinate the psychosocial needs of residents and their families in collaboration with the Nursing Department.

• Bachelor’s Degree in Social Services
• Certification as a CSW (Certified Social Worker) by the state of Missouri
• Knowledge of community resources
• Experience in Health Care Social Work
• 2-5 years of acute or long-term care experience preferred
• MDS experience a plus
• Experience in long-term care social work also a plus
