We are seeking an experienced and motivated Lead Practice Nurse for 30-37.5 hours per week to join our friendly NHS practice and play a key role in shaping and supporting our nursing team at Manor Park Surgery.

Main duties of the job

The postholder will be responsible for leading the delivery of nursing services at the practice, working as part of the practice multidisciplinary clinical team and delivering nursing care within their scope of practice to the patient population. This will include, but is not limited to assessing, examining and managing patients with chronic disease, wound care, cervical cytology, family planning, travel health and child & adult immunisation.

The postholder will also have responsibility for leading and managing the nursing team in collaboration with the Senior GP Partner. The Lead Nurse will provide support, training and mentorship for the clinical team, ensuring the appropriate skill mix to meet the nursing needs of the practice population. They will be involved in the strategic development of nursing services at the practice, developing and reviewing clinical policy and procedure, participating in audit, case finding and clinical meetings, prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.

Manor Park Surgery is a GP surgery based in Bramley, Leeds.

We are a four GP partner training practice serving a population of circa 14,000 patients, and we use Systm One. We offer training for medical students, nursing students and GP registrars. We are a forward thinking, multi-disciplinary teamand we work supportively with our Care Navigation and Administration teams. We havea purpose-built, two storey building with on-site parking facilities and we are rated good by the CQC.

Job responsibilities

Management and Leadership

Actas a clinical leader inthe delivery of nursing services across the Practice; actingas accountable clinical nursing lead inthe care provided to patients bythe nursing team

As part of the senior management team, develop close relationships with partners, colleagues and other members of theteam.

Providing effective line management to practice nurses and Health Care Assistants.

Actively provide the nursing perspective with attendance to relevant internal and external committees or groups

Actively maintain, monitor and audit the qualityof care provided, with reference to clinical governance, risk or issues management, documentation and patient records

Workwith the Partnership and Clinicare Health Support Services management teamin clinical service redesign andother transformation work streams as appropriate

Ensure that there is effective communication between the nursing team and other teams within the practice

Implement evidence-based recommendations and innovations to improve care

Actively takepart in the recruitment, selection, training and appraisal of practice based nursing posts.

Be involved in the teaching, mentoring and coaching of staff members, nursing students and members of the multidisciplinary team as appropriate.

Setup, maintain and organise agreed training and development programmes in line with Practice priorities

Develop and embed a model of clinical supervision and reflective practice on an individual or group basis.

Support HR escalation of performance issues within wider nursing team as appropriate.

Be the practice representative for nursing team for the West Leeds Primary Care Network or other external agencies as appropriate

Support the ongoing development of the multi-disciplinary clinical team across the practice that will enable provision of innovative,high quality services to our patients

Tosupport and influence the role of our nursing team within any clinical service redesign plansboth within the Practice and also within the wider Pudsey Primary Care Network

Provide leadership inareas related to infection prevention and control and emergency equipment, including being the named

Infection Prevention and Control lead for the Practice, ensuring compliance and oversees audit as necessary
