Advanced Clinical Practitioner

An excellent opportunity to join our friendly, innovative, and expanding GMS practice in a beautiful semi-rural location between York and Leeds.

We are seeking a dedicated experienced ACP to enhance our supportive team due to retirement of our existing clinican.

Our clinical roles include, capped appointment numbers, 15-minute F2F appointments, low home visit rates, access to paramedics visiting,first contact physio & mental health practitioner.

We support our team with a daily clinical meet and have low clinical staff turnover.

We are also part of a well-supported PCN (Tadcaster and Rural Selby PCN).

We have always been high QOF Achievers, participate in enhanced services, dispense to nearly half our population and have an onsite Pharmacy.

We also have branch sites in Micklefield and Thorpe Willoughby.

South Milford Surgery is a 3-partner, semi-rural practice, situated in the village of South Milford located within the District of Selby. We also serve our 2 branch surgeries at Micklefield and Thorpe Willoughby. The practice serves approximately 11,000.

The practice operates under a Personal Medical Services contract with Vale of York, NHS Humber & North Yorkshire Integrated Care Board (ICB).

We are seeking an experienced Advanced Clinical Practitioner (ACP) to join our clinical team.

You will work autonomously to:

• Assess, diagnose, and manage patients with acute, chronic, and complex conditions
• Prescribe and deliver treatment plans, therapies, and lifestyle advice
• Provide chronic disease management and health promotion
• Support safeguarding, clinical emergencies, and multidisciplinary care
• Contribute to clinical governance, audits, and service improvement

• The individual must be confident, autonomous and patient focused who is committed to professional standards.

• Someone with a caring attitude, who has an ACP Masters Qualification, registered with appropriate regulatory body and is an independent prescriber.
