- Clinical Assessment & Prescribing:
Conduct comprehensive assessments for individuals accessing substance misuse services and independently prescribe in line with clinical guidelines - Care Planning & Reviews:
Develop, monitor, and review personalised care plans, supporting service users to achieve recovery goals - Substance Misuse & Harm Reduction:
Provide harm reduction advice and support detoxification and stabilisation plans - Physical & Mental Health Monitoring:
Assess and monitor associated health conditions, facilitating access to primary and specialist services - Safeguarding &
Risk Management:
Identify safeguarding concerns and manage risks to ensure service user safety - Multi-Disciplinary & Partnership Working:
Collaborate with internal teams and external partners to deliver holistic care - Clinical Governance & Professional Practice:
Maintain high standards of clinical record keeping and contribute to service improvement initiatives
- Registered Nurse, Pharmacist, or Allied Health Professional with an Independent Prescriber qualification (V300)
- Current registration with NMC, GPhC, or HCPC, with evidence of continuing professional development and revalidation
- Essential experience within substance misuse, community health, or mental health services, including independent prescribing
- Desirable knowledge of opioid substitution therapy and recovery-focused practice
