Job Summary

The organisation has an exciting opportunity in the Primary Care Network for a clinical pharmacist to join the team. The role will support the development of medicines optimisation services within the Network. Morley has a well-established PCN made up of 6 GP practices covering a population of 63,000. It is in a vibrant market town 5 miles outside of Leeds. The role may be working across any site within the PCN.

Job Description

Job responsibilities

Undertake clinical medication reviews with patients and produce recommendations for Nurses and/or GP on prescribing and monitoring. Review the ongoing need for each medicine, monitoring requirements, and support patients with their medicines to ensure best use. Make appropriate recommendations to GPs for medicine improvement. Perform risk stratification to identify cohorts at high risk of harm from medicines. Review medicines most associated with unplanned hospital admissions and implement changes to reduce risk.

Provide medicines support for patients with questions and concerns about their medicines.

Manage medicines at the transfer of care, reconciling medicines after discharge and ensuring continuity of supply for high-risk groups. Answer medicine-related enquiries from GPs, the wider MDT, and other staff. Work with the Lead PCN Clinical Pharmacist, Pharmacy Technician, Prescription Teams and Dispensary Teams, supervising and advising when appropriate. Collaborate with the extended PCN Pharmacy Team to deliver the PCN shared agenda.

Responsibilities

– Prescribing Quality and Safety

Support repeat prescribing policy within each Practice. Manage repeat prescribing reauthorisation by reviewing requests and monitoring review dates. Ensure appropriate monitoring tests are in place. Analyse and present medicines data to support decision making. Undertake clinical audits and implement changes in response to MHRA alerts and guidance. Implement local and national guidelines and formulary recommendations. Monitor prescribing against the local health economy's RAG list and advise GPs on medicines that should be prescribed by Hospital Doctors or subject to shared care.

Liaise with ICB Pharmacists and Heads of Medicines Management to share learning. Foster links with services across the PCN and neighbouring networks and explore opportunities for collaboration with stakeholders including patients, GPs, Nurses, Pharmacists, and hospital staff.

Professional Development

Work with the line Manager to undertake ongoing personal and professional development, attend CPD sessions with the MDT, and participate in clinical, prescription and dispensary meetings. Adhere to policies and ensure confidentiality, safeguarding, information governance, health and safety. Seek regular clinical supervision and review yearly progress with clear plans aligned to priorities.

Education, Qualifications and Experience

Qualifications

Essential

  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Mandatory registration with the General Pharmaceutical Council.
  • Holds an independent prescribing qualification.
