Enhanced Care Recovery Practitioner – Forward Leeds

– 37 hours Monday‑Friday, Permanent – Salary £25,110 – £32,090 per annum.

Role OverviewAs an Enhanced Care Recovery Coordinator, you will support higher‑need service users who are unable to progress within mainstream services. You will conduct outreach, home visits, and facilitate engagement with specialist services, providing intensive support and interventions that improve outcomes.
Key Responsibilities- Assess and engage individuals with multiple needs, using flexible outreach to reduce barriers to mainstream support.
- Manage a caseload, reviewing ongoing care, and liaising closely with specialist workers and family/carers.
- Conduct home visits and support clients to appointments across disciplines.
- Exercise excellent therapeutic and communication skills when dealing with service users, carers, and agencies.
- Collaborate with the multi‑disciplinary team and attend reviews, team meetings, and supervision.
- Passion for making a difference and commitment to fairness for all.
- Knowledge of brief interventions, Motivational Interviewing, Psychosocial Interventions, Cognitive Behavioural Therapy, Harm Reduction, and multidisciplinary working.
- Valid driving licence and access to a vehicle.
- Experience working with adults with complex needs is desirable.
