We are recruiting a Diabetes Clinician to work alongside our established team to provide high class clinical care to our patient population.
As a vital member of our team, you will play a pivotal role providing and delivering prescribed programmes of patient care.
Main duties of the jobThe Diabetes Clinician will provide high-quality, patient-centered diabetes care for our patient population. The role is open tosuitably qualified General Practitioners, Nurses, or Pharmacists with advanceddiabetes expertise. The post holder willlead on the management of people with diabetes, including insulin initiationand titration, optimisation of therapies, and support for the wider practiceteam to improve diabetes outcomes in line with national and local guidelines.

We are a Personal Medical Services (PMS) practice offering
Primary Care Services for the diagnosis and prevention of disease. We help patients to manage their health and prevent illness. Our GPs assess, diagnose,treat and manage illness. They carry out screening for some diseases and promote general health and wellbeing. Our GPs act as a patients advocate,supporting and representing a patients best interests to ensure they receive the best and most appropriate health and/or social care.
The Practice has a current list size of 30,000 patients andwe are part of the West Yorkshire Integrated Care Board (formerly NHS Wakefield Clinical Commissioning Group).

Person Specification Experience- Proven experience in insulin initiation and titration within a primary care or community setting.
- Significant experience managing patients with diabetes in clinical practice.
- Sound knowledge of current diabetes guidelines and best practice
- Demonstrate an understanding of audit
- Compiling procedures and clinical guidelines
- Relevant experience of working within a primary care or and community-based environment
- Involvement in diabetes service development or quality improvement projects.
- Strong clinical assessment and decision-making skills.
- Excellent communication skills with patients and healthcare professionals.
- Ability to work autonomously while contributing effectively to a multidisciplinary team.
- Commitment to patient-centered care and reducing health inequalities.
- Competent IT skills, including use of clinical systems and data recording.
- Punctual timekeeper and a positive role model
- Ability to work core hours
- Flexibility for cover
- Ability to travel to meetings
- Registered healthcare professional as GP, Nurse, or Pharmacist with current professional registration (GMC / NMC / GPhC).
- Independent Prescribing qualification (for Nurses or Pharmacists).
