Beeston Primary Care Network (PCN) has a vacancy for a highly motivated and enthusiastic Clinical Pharmacist to join the team. The team is well integrated into the PCN and works closely with Middleton and Hunslet PCN, sharing a joint Executive Team across both PCNs. Their work is highly valued and recognised by all member practices. The PCN roles are well supported by the Clinical Director.

Primary Care Networks (PCNs) form a key building block of the NHS long‑term plan. They bring general practices together to work at scale, improve recruitment and retention, manage financial and estates pressures, provide a wider range of services to patients and integrate with the wider health and care system. Middleton and Hunslet PCN is a large PCN with a patient list of about 33,000 patients, providing clinical support to 10 care homes and a workforce that offers in‑house multi‑disciplinary team working.

Job Description Main Tasks – Clinical
  • To improve the outcomes for patients from their medicines and reduce the risk of harm. This includes improving medicines safety and medicines management processes across the PCN such as actioning MHRA alerts and reconciling medicines upon transfer of care.
  • To carry out structured medication reviews (SMRs) to optimise medication for complex patients, especially frail, polypharmacy and care home residents. This may involve home visits for housebound patients jointly with care coordinators in the frailty team.
  • Contribute to delivering the medicines aspects of the investment and impact fund (IIF) which is part of the PCN directed enhanced service (DES).
  • Join established MDTs locally to deliver proactive, personalised and coordinated health and social care, with partnership working across all care sectors and effective communication.
Main Tasks – Technical & Administrative
  • Support the PCN multi‑disciplinary team to ensure efficient medicines optimisation processes are followed.
  • Implement efficient ordering and return processes and reduce medication wastage.
  • Provide training and support on the legal, safe and secure handling of medicines, including implementation of the Electronic Prescription Service (EPS).
  • Promote Electronic Repeat Dispensing (eRD) and online ordering.
  • Develop relationships with other pharmacy professionals and multi‑disciplinary team members to support integration across health and social care, including primary care, community pharmacy, secondary care and mental health.
  • Support practice reception teams in streaming general prescription requests, allowing GPs and clinical pharmacists to review the more clinically complex requests.
  • Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.
  • Support the PCN to deliver on the Quality Improvement Plans, QOF and locally commissioned enhanced services.
  • Support the PCN in reviewing and developing practice policies for CQC requirements.
Health & Safety
  • Use personal security systems within the workplace according to PCN guidelines.
  • Identify the risks involved in work activities and undertake them in a way that manages those risks.
  • Use effective training to update knowledge and skills.
  • Use appropriate infection control procedures, maintaining a tidy and safe work area free from hazards and practicing effective hand hygiene.
  • Provide advice on the correct and safe management of specimen processes, including collection, labelling, handling, use of correct containers, storage and transport arrangements.
  • Use correct personal protective equipment (PPE) in both routine and extraordinary circumstances.
  • Safely use sharps, and store and dispose of them properly.
  • Report potential risks identified.
