Job Classification
Social Counselor 2 – Non‑Exempt – Monthly salary $3,631–$5,434.
PurposeProvide case management, counseling, and referrals to Health Department patients and families experiencing medical or socio‑economic difficulty.
Responsibilities- Interview clients, assess needs, develop service plans, and follow up to ensure referrals are completed.
- Maintain accurate case documentation and prepare required paperwork.
- Coordinate with community agencies and Health Department staff to promote available services.
- Conduct home visits and participate in community outreach events.
- Operate a motor vehicle and general office equipment as required.
- Bachelor’s degree in Social Work, Sociology, Human Services, or related field.
- Associate’s degree plus two years of related experience may substitute for the bachelor’s degree.
- Valid Tennessee driver’s license required.
