LPN – Licensed Practical Nurse – $2,500 Sign‑On Bonus!

Open Arms Care is hiring a Licensed Practical Nurse (LPN) for full‑time positions with a base pay range of $27.00/hr to $30.00/hr
. This role includes a sign‑on bonus of $2,500 plus additional benefits.

Provide nursing and health care services to individuals in accordance with the organization’s policies and the state’s Nurse Practice Act. Provide training to clients and employees, and handle pre‑employment screening and committee participation.

• Provide training to all employees and clients.
• Evaluate emergency situations for clients, determine appropriate actions and follow-up.
• Accountable for all scheduled and controlled substances per policy and state guidelines.
• Maintain security and cleanliness of medication storage areas.
• Record and report all vital signs and clinical observations.
• Administer treatments and medications as ordered, assist physicians as required.
• Rotate taking call and advise other LPNs.
• Notify Health Services Director and physician on‑call of any medical incident.
• Transcribe physician orders accurately and efficiently.
• Ensure adequate drug and pharmaceutical stocks are maintained.
• Follow facility, pharmacy and state procedures for ordering and receiving medications.
• Provide and document all medical and nursing care provided.
• Ensure client safety at all times.
• Assist direct‑care employees to provide high‑quality care.
• Participate in developing Individual Support Plans (ISP) through interdisciplinary meetings.
• Complete desensitization programs for dental/medical appointments.
• Perform other duties as assigned.

• Associate’s degree in Nursing or graduate of an LPN program (required).
• Active State of Tennessee Licensed Practical Nurse license (required).
• CPR certification within 30 days of employment (required).
• Certified in Professional Crisis Management (PCM) within 60 days of hire (required).
• Valid driver’s license (required).

• Exceptional written and verbal communication skills.
• Strong organizational and time‑management skills.
• Accountability, adaptability, teamwork, and a strong sense of responsibility.

• Ability to lift repetitively a minimum of 50 pounds.
• Ability to sit or stand for long periods.
• Ability to physically assist and interact with individuals with intellectual and developmental disabilities.
• Ability to check adaptive equipment for functionality.
