Case Management Nurses coordinate long‑term care for their patients. Their goal is to treat patients at optimal times to keep them healthy and out of the hospital. As a Case Management Nurse, you’ll coordinate care for specific patient groups, work with specialties such as HIV/AIDS, cancer, geriatrics or pediatrics, research the latest treatments, and partner with insurance companies to secure the best, most cost‑effective care possible.
Responsibilities- Coordinate comprehensive, long‑term care plans for assigned patients.
- Collaborate with specialists, hospitals, and community resources to ensure seamless transitions.
- Review treatment protocols and stay current on evidence‑based practices.
- Advocate for patients’ medical and insurance needs to secure optimal care.
- Minimum of 1 year of current work experience in CASE MANAGEMENT.
- New Hampshire State Healthcare Provider license or willingness to obtain one.
- BLS
