Lebanon Center (Lebanon, NH). Position is Full‑time at the Genesis Healthcare Lebanon Center.

$44.00/hr – $48.50/hr

• Ensure appropriate reimbursement for patient care services.
• Participate in the Patient Assessment Process by completing MDS and CAAs.
• Assist in developing individualized patient care plans and identifying patient needs and projected outcomes per federal and state regulations.
• Contribute to the nursing center interdisciplinary Resident Assessment Instrument (RAI) process, following guidelines for completion, encoding, and transmission.
• Identify changes in patient condition with the interdisciplinary team and coordinate scheduling of indicated assessments.
• Assist with Activities of Daily Living training for new hires and ongoing education for existing staff.
• Attend educational programs to maintain knowledge of current RAI/Prospective Payment System regulations.

• Registered Nurse, currently licensed by the state and in good standing.
• Prior experience in a hospital, long‑term facility, or other healthcare setting.
• Experience completing MDSs and Care Plans.
• Knowledge of general, rehabilitative, and restorative nursing and medical practices, including regulations and guidelines for long‑term care.
