In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - New London CT

Signify Health is seeking a Part Time Clinician (NP or PA) to provide In-Home Health Evaluations to patients in the comfort of their homes.

• Conduct in-home visits lasting approximately 45 minutes to provide a medication and medical history review, a physical evaluation, and, if ordered, additional diagnostic tests.
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Be flexible to travel locally and within licensed states, as needed.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products such as spirometry, DEE, bone density screening, etc.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Able to cover the primary locations:
  New London and surrounding area.

Anticipated weekly hours: 40

Base pay range: $46.00/hr – $99.00/hr
