Provide primary and palliative care to homebound patients in Lawton, OK. Work within a collaborative team, applying medical principles under physician supervision to assess, diagnose, and treat patients with chronic illnesses.
What You'll Do- Deliver high‑quality care to primary and palliative patients in and around Lawton, OK.
- Conduct physical examinations, review and update medical histories, and develop comprehensive care plans.
- Communicate effectively with office staff and clinical operations across internal platforms.
- Provide patient‑centered care for a panel of patients.
- Develop impressions, diagnoses, and treatment plans.
- Build trust and offer consultation to patients, families, and caregivers during visits.
- Order diagnostic testing as medically necessary.
- Prescribe medications, supplies, and appropriate interventions.
- Board Certified with a valid NP or PA certification in the State of Oklahoma.
- Strong clinical skills for accurate assessment, diagnosis, and treatment.
- Positive demeanor and excellent interpersonal skills.
- Ability to problem‑solve and operate independently.
- 1–2 years of primary‑care experience with geriatric patients (hospice experience highly preferred).
