As we continue to expand, we are seeking a skilled and dedicated Registered Nurse Case Manager to join our team.

Job Description for a RN Case Manager:

We are currently seeking a Registered Nurse Case Manager to join our team!

The Registered Nurse will be responsible for providing comprehensive case management services to our clients, ensuring they receive the highest level of care and support. This position offers flexibility and the opportunity to make a meaningful impact in the lives of others. This job is a great part time opportunity and provides the chance to grow with a start‑up franchise. Each assessment, along with the corresponding paper work, is expected to take 1.5 to 2 hours to complete.

The number of assessments will vary from week to week. The Nurse will also perform periodic home visits to ensure that the caregiver is adhering to the care plan. Other services provided by the RN Case manager will include case management, support, coordinating with families and other health care professionals etc.

The successful candidate will be responsible for providing their own transportation to and from each appointment. Our territory is Lawrenceville, Suwanee, Grayson, Snellville and Duluth. The RN Case Manager reports to the owner.

Responsibilities of the Registered Nurse
  • Complete new client admissions, including conducting initial assessments, obtaining medical histories, and developing individualized care plans.
  • Meet with families and clients to discuss the benefits of home care and address any questions or concerns they may have.
  • Develop the initial care plan based on client needs and preferences.
  • Oversee caregivers' adherence to the care plan, including conducting regular visits to client homes to assess care quality and provide guidance and support to caregivers.
  • Ensure all services provided are in compliance with state regulations and agency policies and procedures.
  • Collaborate with other members of the healthcare team, including physicians, therapists, and social workers, to coordinate care and ensure continuity of services.
  • Maintain accurate and up‑to‑date documentation of client care, including progress notes, care plans, and medication records.
  • Provide education and support to clients and their families on health management, medication management, and safety practices.
  • Participate in case conferences and team meetings to discuss client progress, identify areas for improvement, and develop strategies to enhance care delivery.
  • Act as a liaison between clients, caregivers, and other healthcare professionals to facilitate communication and ensure the highest level of care and support.
  • Coordinate and oversee the implementation of care plans, including coordinating with other healthcare professionals and community resources as needed.
  • Provide ongoing monitoring and evaluation of client progress and make adjustments to care plans as necessary.
  • Administer Personal Care Assistant (PCA) Part II & Part III practical exams to Caregivers.
Qualifications
  • Current and valid RN license in the state of Georgia.
  • Current valid Drivers License.
  • Current valid CPR certificate.
  • Current valid TB test result.
  • Minimum of 2 years of experience in nursing, preferably in home care or a related field.
  • Strong assessment, communication, and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassionate and empathetic approach to patient care.
  • Reliable transportation and willingness to travel to client homes as needed.
