The Nurse Manager, Care Management Program is a key leader within Population Health, overseeing daily operations of a comprehensive care management program serving pediatric, adult, and maternal populations. The role ensures high‑quality care coordination for patients in value‑based care models and maintains compliance with state, federal, and contractual standards.
The Nurse Manager leads an interdisciplinary team of RN Care Managers, Behavioral Health Care Managers, and Community Health Workers. The position requires expertise in care management regulations, workflow optimization, and performance improvement.
Key Responsibilities- Oversee daily program operations and ensure regulatory compliance
- Develop and revise workflows based on contract updates and program evaluations
- Monitor performance metrics, dashboards, and caseload distribution
- Use data insights to improve efficiency, quality, and patient outcomes
- Partner with Population Health leadership on quality initiatives and staff development
- Conduct chart audits, supervision, and performance evaluations
- Communicate policy and workflow updates to staff
- Deliver training to support competency and best practices
- Maintain proficiency in care management technology platforms
- Lead clinical rounds and participate in interdisciplinary care meetings
- Manage staffing needs including recruitment, onboarding, scheduling, and coverage planning
- Bachelor’s degree in Nursing and active RN license
- Case Management certification preferred
- 3–5 years of management experience in care management or population health, ideally in value‑based or managed care
- Experience with Medicaid or complex medical, behavioral, and social needs populations
- Proven ability to lead interdisciplinary teams and collaborate with providers and community partners
