Overview
Family Connector (Mental Health Services) — The ACCS Family Connector will ensure that each person served has a stable support network of family members, community members and friends as they develop life skills to live independently. The ACCS Family Connector will work with adults with psychiatric conditions and those who identify as their supports to provide outreach, information, referral and direct support services.
In this role you will deliver interventions targeted at skills building including activities of daily living, medication administration, symptom reduction, health and wellbeing, employment, school, socialization and recreation. You will work to involve family and friends in the person served’s life and Treatment Plan interventions when appropriate, foster a welcoming environment for families/supporters, orient them to services, and engage their participation toward the person’s ACCS goals.
You will act as the primary liaison between team services and help create and maintain the team’s Family Advisory Council.

- Bachelor's degree in human services, Master's degree in Human Services, or equivalent experience
- 2 to 3 years of experience working with adults with psychiatric conditions
- Must have access to a vehicle and be willing to use it to transport Persons Served; valid US driver’s license
- Preference for bilingual/bicultural applicants and applicants with lived experience of psychiatric conditions
- Age 21 or older; able to pass CORI, driving record, references, educational and professional licensure checks, and multi-state background check
- CPR, First Aid within 2 weeks of employment; MAP certification within 150 days
- Experience as a caregiver of a youth or adult with special needs; preferably with psychiatric conditions
- Relevant educational background with one year of experience with children/adolescents/TAY or adult populations
- Experience navigating family-serving systems and teaching family members involved with those systems
