Job Responsibilities
- Partner with residents and care providers to maximize functional abilities and independence in daily activities.
- Support and promote our mission and values, making a real impact on residents’ lives.
- Perform evaluative and therapeutic procedures under physician orders while respecting safety guidelines.
- Evaluate residents and establish personalized therapeutic plans with functional goals and modalities.
- Collaborate with residents and primary caregivers to tailor therapy goals and enhance care.
- Consult with healthcare team members about residents' plans of care and discharge needs.
- Monitor treatment effects, adjust plans, and ensure residents achieve their goals.
- Document interactions in the Electronic Medical Record clearly, allowing easy access for the care team.
- Recommend and instruct residents on the use of appropriate assistive devices.
- Provide education to residents and caregivers, documenting teaching outcomes.
- Refer residents to Physical Therapist Assistants as needed, ensuring a cohesive care experience.
- Understand and comply with Medicare and other reimbursement regulations.
- Maintain current clinical skills through learning activities and provide in-service education to staff.
- Engage in developing new rehabilitation programs and services that enhance resident care.
- Stay informed on professional issues and share pertinent insights with your team.
- Supervise assistant-level staff, ensuring compliance with training standards and best practices.
- Mentor new staff during orientation, completing orientation checklists and competencies.
- Ensure cleanliness and maintenance of office and clinical areas, adhering to infection control procedures.
- Maintain required licenses, certifications, and complete compliance courses on time.

- Proficient in Microsoft Office applications.

- Fluent in English (reading, writing, and understanding instructions).
- Bachelor’s degree required;
Master’s or Doctorate preferred. - Active PT license in the applicable state.
- Basic Life Support (BLS) certification preferred.
- Prior experience in long-term care or skilled nursing facility is preferred.
- Equivalent combination of education and experience may substitute for requirements.
- Physically able to perform tasks involving sitting, standing, walking, bending, and lifting.
- Ability to lift up to 50 pounds (2-person lift over 50 pounds).
- Frequently lift/carry up to 20 pounds. Expect some stress due to high responsibility for quality care.
