Under the direction of the Community Health Access Manager, the CSS Social Services Supervisor provides leadership and oversight of daily operations within the Community Support Services (CSS) Department. This role ensures high-quality service delivery, care coordination, staff development, program implementation, and grant compliance. The Supervisor also oversees the Outreach & Enrollment (OE) program, ensuring access to Mass Health applications and insurance assistance services.
ResponsibilitiesOperations & Staff Management
- Oversee daily CSS operations, including staff scheduling, case management, and referral tracking
- Supervise, evaluate, and support staff performance
- Conduct chart reviews to ensure accurate assessments and appropriate referrals
- Collaborate with clinical teams to meet patient needs
- Establish and update CSS policies and procedures
- Monitor EMR templates and address scheduling issues
- Support strategic planning and departmental goals
- Monitor program performance and analyze data
- Oversee SNAP-certified CBO projects, mobile markets, Access to Care, and wrap-around services
- Implement workflows for social needs screenings and referrals using Unite Us and other platforms
- Oversee the Outreach & Enrollment program, including Mass Health applications and renewals
- Conduct monthly supervision and group meetings
- Approve time-off, payroll, and performance reviews
- Support recruitment, hiring, onboarding, and orientation
- Ensure staff proficiency in Athena, SharePoint, and EVS
- Develop and deliver training curricula, manuals, and competency assessments
- Track staff training completion and provide feedback
- Support new hires after training
- Serve as Lead Certified Application Counselor (CAC)
- Represent GLFHC at community events and meetings
- Maintain partnerships with community organizations
- Ensure patient follow-up and access to enabling services
- Oversee insurance enrollment and transportation support (PT-1)
- Minimum of 3 years’ experience in the Human Services field, including at least 2 years of direct supervisory experience
- Knowledge of community and social service resources
- Strong organizational and data management skills
- Certified Application Counselor (CAC) preferred
- Excellent verbal and written communication skills
- Valid Massachusetts Driver’s License and reliable transportation required
- Bachelor’s degree in Human Services, Public Health, Healthcare Administration, or a related field preferred (combination of education and experience considered)
- Bilingual/Bicultural (English/Spanish) strongly preferred
- Community Health Worker Certificate (must be completed within 12–18 months of hire)
- CAC Certification (must be obtained within 6 months of hire)
- BLS Certification required
