The Mass General Brigham Medical Group is deeply committed to health equity and to breaking down barriers to care for our most vulnerable communities. Rooted in research and community need, we are proud to announce the launch of a new Medicaid-focused clinic within our Riverwalk Internal Medicine practice in Lawrence, MA. This program is designed to expand access to affordable, high-quality care for approximately 800 Medicaid ACO patients who have not previously received services through our Medical Group, as well as existing Medicaid ACO patients with urgent or same-day medical needs.
We are seeking a full-time, 40-hour Social Worker to support the Medicaid Clinic! The clinic hours will be Monday-Friday from 8:30 AM to 5:00 PM. We are located at 500 Merrimack Street in Lawrence, MA 01843.

** Summary  The Social Worker is responsible for performing social work assessments and interventions as needed for inpatients, outpatients, and emergency department patients. They are dedicated to patient and family-centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate, and ethical problem-solving. As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations.

Social Workers are responsible for working collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.
Yes    Essential Functions  
-Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
-Formulates biopsychosocial assessment, disposition, and treatment plans.
-Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
-Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
