Minimum Availability Requirement:
Must be able to work a minimum of 10 hours every 90 days
Visiting Nurses Association (VNA) is seeking an experienced and compassionate PRN Registered Nurse (RN) - Hospice Start of Care (SOC) to provide skilled nursing services to patients in their homes. This role is responsible for completing comprehensive Start of Care, visits, while supporting high-quality, patient-centered care in accordance with physician orders and regulatory requirements.
Key Responsibilities- Conduct comprehensive nursing evaluations and routine skilled visits as assigned
- Develop, implement, and update individualized plans of care
- Perform skilled nursing interventions, assessments, and patient education
- Collaborate with physicians, therapists, social workers, CNAs, and the interdisciplinary care team
- Ensure compliance with Medicare, state, and agency regulations
- Complete documentation accurately and within required time frames
- Participate in required compliance training and departmental development activities
- Admission (SOC): $150.00 per visit
- RN Visit: $75.00 per visit
- Current, unrestricted Registered Nurse (RN) license in the State of Kansas
- Minimum of one (1) year of RN experience required; hospice experience strongly preferred
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
- Strong assessment, documentation, and time-management skills
- Ability to work independently while collaborating with a multidisciplinary team
