Un hôpital universitaire de renom à Lausanne est à la recherche d'un(e) infirmier(e) - Case manager pour ses consultations ambulatoires. Ce poste, à 100 %, implique le suivi de patients présentant des troubles psychiques et nécessite un diplôme d'infirmier-ère HES ainsi qu'une expérience en psychiatrie d'au moins 3 ans. Vous travaillerez en équipe pluridisciplinaire et serez responsable de l'identification et de la planification des soins.
