VOS PRINCIPALES MISSIONS :
- Assurer des soins à domicile personnalisés et de qualité en respectant les besoins et attentes des patients.
- Être le relais essentiel entre le corps médical, le patient et sa famille : écoute, information et accompagnement.
- Participer à l'évaluation clinique et sociale des patients afin d'adapter leur prise en charge.
- Travailler en étroite collaboration avec une équipe pluridisciplinaire pour proposer des solutions adaptées.
- Veiller au confort émotionnel et physique des patients avec bienveillance et professionnalisme.
