Responsibilities- Provides dementia care including adapted ADLs, meaningful engagement and communication to residents following a person-centered individual service plan. Treats residents with respect and dignity, recognizes individual needs, and encourages independence. Fosters a purposeful and engaging culture throughout the community.
- Assists residents and provides input regarding the care plan and daily living activities, including bathing, dressing, grooming, toileting, transferring, and getting to programs and meals according to the individual service plan. Encourages residents to be as independent as possible.
- Participates in team meetings or discussions with community leadership.
- Knows residents’ Life Story, preferences, historical routines and interests.
- Serves meals in the dining room and may assist in meal preparation following dementia-friendly menus. Encourages proper diet and fluids while respecting residents’ right to free choice. Records changes in eating habits to the supervisor.
- Initiates and/or leads Daily Path engagement programs and supports socialization and participation in person-centered activities.
- Maintains residents’ records daily, including engagement interests, incidents, observations, and changes in physical or emotional condition, reporting to the supervisor.
- May assist with medication as permitted by state regulations, following physician orders and medication procedures.
- Maintains a clean, safe, and orderly environment; performs general housekeeping including resident laundry, bedrooms, dining areas, living spaces, and bathrooms.
- Responds promptly to resident requests for assistance and follows emergency procedures.
- Monitors resident location and notifies supervisor if a resident cannot be located.
- Encourages teamwork through cooperative interactions with coworkers.
Education and Experience
High school diploma or GED and three to six months related experience preferred, or an equivalent combination of education and experience. Knowledge of dementia, particularly Alzheimer’s type.
Certifications, Licenses, and OtherSpecial Requirements
May need to possess current state certification and follow regulations to maintain certification, in accordance with state law.
Physical Demands and Working Conditions- Standing, walking, sitting, use of hands, reaching, climbing or balancing, stooping/kneeling/crouching, talking or hearing, tasting or smelling
- Lift up to 50 pounds
- Vision, and ability to interact with coworkers, residents or vendors
- Occasional weekend, evening or night work; on-call as needed
- Possible exposure to communicable diseases, infections, drugs, chemicals, and biological hazards
- Exposure to latex and blood-borne pathogens
- Subject to injury from falls, burns, odors, or equipment; travel occasionally
Ability to follow written or verbal instructions, read and comprehend simple information, and communicate effectively in English. Demonstrates good judgment in emergency situations and seeks assistance when needed.
