What to Expect (Job Responsibilities)
  • Coordinate care for individual clients using assessment, care planning, implementation, coordination, monitoring, and evaluation
  • Conduct in‑depth health risk assessments and comprehensive needs assessments
  • Develop and communicate care plans, serving as the point of contact for service delivery
  • Monitor and evaluate the effectiveness of care plans and interventions
  • Advocate for members’ care needs by identifying and addressing gaps in care
What is Required (Qualifications)
  • 3–5 years of experience in Social Work, Nursing, or a healthcare‑related field, or relevant experience in lieu of a degree
  • Experience in utilization management, quality assurance, home or facility care, community health, long‑term care, or occupational health
  • Strong analytical skills for trend analysis based on decision support systems
  • Effective verbal and written communication skills
  • Valid driver's license in the state of employment
How to Stand Out (Preferred Qualifications)
  • Associate or Bachelor's degree in a relevant field
  • Certification as a Certified Case Manager (CCM) or Licensed Clinical Social Worker (LCSW)
  • Registered Nurse (RN) license in a state or compact state
  • Business management skills, including cost/benefit analysis and negotiation
  • Knowledge of community resources for referral coordination
