Position Overview:

We are seeking a passionate and dedicated Licensed Registered Nurse (RN) to join our home health care team. Responsibilities:

• Provide skilled nursing care, including wound care, medication management, and symptom management.

• Educate patients and their families about their conditions, medications, and self-care techniques.
• Coordinate care with other healthcare providers and community resources to ensure continuity of care.
• Document patient assessments, interventions, and outcomes accurately and timely in accordance with regulatory standards.

Qualifications:
