Nurse Case Manger

Pay Range

$40–$63/hour (based on experience)

The Nurse Case Manager coordinates patient care across the healthcare continuum using a collaborative, multidisciplinary approach. This role assesses, plans, and evaluates patient needs while serving as an advanced clinical resource for patients, families, providers, and staff.

• Graduate of an accredited nursing program
• Minimum 3 years acute care RN experience, including 1 year in Case Management, Discharge Planning, or Utilization Review
• Master’s degree in Nursing (case management focus) may substitute for experience
• BLS (AHA)

• 3+ years Case Management experience in acute care
• Pediatric Case Management experience preferred
• ED and/or Utilization Review experience a plus
• Case Management certification (CCM, ACM) preferred
• Membership in CCMC, ACCM, or ACMA a plus

• Case management principles, care coordination, utilization review, and reimbursement guidelines
• Strong clinical assessment, chart review, communication, and computer skills

Ability to work in a clinical setting with prolonged standing/walking; occasional lifting up to 10 lbs; effective vision, hearing, and communication skills.
