, you will lead discharge planning, improve transitions of care, and manage utilization for hospitalized health plan members in the Las Vegas and Henderson area.
- Perform patient assessment of all major domains using evidence‑based criteria (physical, functional, financial, psychosocial).
- Develop individualized discharge plans with providers, patients, and caregivers for successful transitions.
- Implement discharge plans involving resources across the continuum.
- Monitor and report variances that may challenge timely quality care.
- Flexible to work onsite at a hospital in the Las Vegas and Henderson area.
- Assess, plan and implement care strategies individualized by patient.
- Use company and community resources to establish a safe case‑management plan.
- Collaborate with patient, family, and providers on care plan.
- Communicate health‑related information to ensure coordinated care.
- Advocate for patients and families as needed.
- Determine appropriate level of care using approved criteria.
- Understand insurance products, benefits, coverage limitations, and regulations.
- Accountable for utilization management benchmarks and quality outcomes.
- Provide health education and coach consumers on treatment alternatives.
- Support consumers in selecting physicians and facilities.
- Coordinate services and referrals.
- Assist with physician visits preparation.
- Assess and triage immediate health concerns.
- Manage utilization through education.
- Identify care gaps to intervene.
- Help members sort benefits and make choices.
- Handle inbound/outbound calls per needs.
- Perform special projects and other duties as assigned.
- Work in Sub‑Acute or Acute Hospital settings.
- Occasional driving up to one hour daily for regular employees.
- Registered Nurse with active, unrestricted license in Nevada.
- 2+ years of adult clinical, managed care, or case‑management experience.
- Intermediate PC proficiency in Windows (MS Word, Excel).
- Valid driver’s license and reliable transportation.
- CCM certification or ability to obtain within 2+ years of employment.
- 2+ years of ER, ICU, IMC/step‑down, or med‑surg experience.
- Case‑management or utilization review experience.
- Experience in a managed care organization.
Knowledge of Inter Qual or MCG.
Hourly pay ranges from $28.27 to $50.48 based on experience and qualifications. Additional benefits include a comprehensive package, incentive and recognition programs, equity stock purchase, and 401(k) contribution.
