Overview
Healthcare in Action serves patients experiencing homelessness through an innovative street medicine approach. We contract with Medicare and Medicaid health plans as well as hospitals and health systems. Street medicine involves delivering medical care via a medically equipped van to individuals in locations such as parking lots, encampments, and parks. Healthcare in Action is launching its first team in the Las Vegas area to improve access to quality holistic care addressing mental, social, and physical health needs.
HIA was established in 2021 as a 501(c)(3) organization and associated medical group. Our teams travel directly to unhoused individuals living in communities across Nevada, providing services where they reside and building trust to support health and housing goals.
The Nurse Practitioner, Street Medicine provides direct care as an independent healthcare provider to people experiencing homelessness in the Las Vegas, NV region.
Responsibilities- Conduct physical exams and assessments, take medical histories, diagnose and document health conditions, perform wellness exams, administer vaccinations, and draw blood for lab tests if necessary. Produce care plans for patients.
- Perform Chronic Care Assessments and organize/complete accompanying chart audits and individualized Care Plans.
- Interpret and evaluate diagnostic tests to identify clinical problems and health care needs; document chronic health conditions and coordinate follow-up with SCAN's local provider network.
- Develop care plans in collaboration with patients, peer navigation, and primary care physicians. Educate and advise on medical issues; consult with physicians and Medical Director as needed; ensure Primary Care Physicians are aware of patient conditions and progress.
- Provide health education to patients and support prevention, health promotion, and management of chronic conditions. Facilitate referrals internally and externally as appropriate.
- Create a trusting environment with providers and patients; maintain a safe, efficient, and professional setting; collaborate with physicians and other health care professionals in clinical management.
- Follow company policies and procedures, including proper handling of lab specimens and biohazardous waste; document visits accurately; report suspicious or illegal conduct and cooperate with investigations.
- Understand the Health Plan’s encounter data submission process and CMS requirements; participate in grievance, audit, continuing education, quality assurance, and quality improvement programs; serve on clinical committees as assigned.
- Lead and coach teams (social workers, peer navigators) and act as liaison to supervising physicians/psychiatrists; practice medicine within the scope of a California Nurse Practitioner license.
- Drive the mobile van as needed and maintain professional behavior in all interactions.
- Other duties as assigned.
- Graduate degree (Master of Science in Nursing or Doctor of Nursing Practice) from an accredited nursing school.
- NPI, DEA license in good standing.
- 5+ years of experience as a Nurse Practitioner in a medical office, field, or hospital setting.
- 1+ years of experience in street medicine, mobile medicine, and/or FQHC.
- Leadership/supervisory experience strongly preferred.
- Fluent in Spanish strongly preferred; experience with homeless/underserved populations strongly preferred.
- Understanding of ICD-10 coding preferred.
- Ability to work under minimal direction.
- Current First Aid/CPR/AED certification.
- Knowledge of efficient interviewing practices and techniques; strong interpersonal and communication skills.
- Ability to work on a team and maintain good working relationships with staff and patients; culturally sensitive to a diverse population.
- Ability to learn and use computer-based applications (MS Office, EMR); knowledge of medical records and documentation requirements; maintain patient confidentiality and HIPAA compliance.
- Ability to remain calm and resourceful in emergencies and to maintain a professional demeanor.
- Ability to work outdoors in austere conditions; ability to climb, stoop, kneel, crouch, and lift up to 15 pounds regularly and up to 50 pounds occasionally.
- Proof of valid driver’s license; ability to drive a company vehicle; must have access to a vehicle and maintain minimum state liability insurance.
- TB screening compliance or proof of negative test within the last year; accommodations available if needed.
- Base salary range: $106,200 to $170,000 annually
