RN Assessment Nurse

Job Summary: SSM Health is seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as an Assessment Nurse for our home care agency in Nevada. As an Assessment Nurse, you will be responsible for conducting comprehensive assessments of patients in their homes, identifying their healthcare needs, and developing individualized care plans to ensure optimal health outcomes.

· Conduct initial and ongoing assessments of patients in their homes, including physical, emotional, and social assessments
· Identify patients' healthcare needs and develop individualized care plans in collaboration with patients, families, and healthcare providers
· Provide education and support to patients and families to promote health, wellness, and disease management
· Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care
· Document assessments, care plans, and patient progress in accordance with agency policies and regulations
· Participate in quality improvement initiatives to ensure high-quality patient care and agency compliance with regulatory requirements
· Maintain accurate and up-to-date records of patient care and services provided
· Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions and coordination Requirements:
· Current Nevada RN license
· Bachelor's degree in Nursing (BSN) or higher
· Minimum of 1 year of experience as a Registered Nurse, preferably in home care or hospice
· Strong assessment and communication skills
· Ability to work independently and as part of a team
· Familiarity with Medicare and Medicaid regulations and guidelines
· Ability to travel to patients' homes in the Nevada area
· Reliable transportation and a valid driver's license
· Current CPR certification
· TB test and background check required Preferred Qualifications:
· Experience with electronic medical records (EMRs) and care coordination software
· Certification in a specialty area, such as wound care or palliative care
· Bilingual or multilingual skills Work Environment:
· Work in patients' homes in the Nevada area
· May be required to work in a fast-paced environment with multiple priorities
· Must be able to lift, push, and pull equipment and supplies as needed
· Must be able to work in a variety of settings, including patient homes, hospitals, and clinics Benefits:
