Licensed Practical Nurse, LPN – Home Visits

- Make home visits to clients within designated territories.
- Administer medications, perform wound care, treatments, and procedures.
- Monitor client conditions and report changes to the Clinical Manager and Client Services Manager.
- Execute physicians’ orders and document them properly.
- Conduct client assessments as needed.
- Coordinate case management.
- Accurately document observations, interventions, and evaluations using electronic medical records on a touch‑pad tablet.
- Current licensed Practical Nurse in Nevada.
- Minimum one year recent verifiable experience as an LPN.
- Graduation from an accredited nursing program, confirmed by transcript or diploma.
- Home care experience strongly preferred.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills; EMR experience preferred.
- Willingness to travel to assigned cases.
