Overview

MDS LPN/LPN (Healthcare)

Responsibilities
  • Administrative Functions: Administer patient assessments, oversee the assessment process, set assessment schedules and assure assessments are done accurately and timely. Coordinate the care plan according to regulatory requirements. Create schedules for Medicare and Medicaid. Manage Medicare coverage for newly qualified patients or send out denial letters. Stay updated on changes in Medicare coverage and determine documents needed for Medicaid reimbursement. Direct the day-to-day functions of nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility.

    Ensure all nursing personnel comply with facility policies and procedures. Periodically review the department’s policies, procedure manuals, job descriptions, etc., and make recommendations for revisions. Plan shifts, services, programs, and activities with assigned nursing staff and support personnel. Maintain the Nursing Service Procedures Manual and ensure compliance by nursing service personnel. Provide written and oral reports/recommendations concerning shift activities as required.

    Coordinate with other resident services to maintain the resident’s total regimen of care. Participate in the facility’s quality assurance program for the nursing service department and in facility surveys as requested. Review and update resident discharge plans as required and assist in planning the nursing services portion of discharge plans. Interpret department policies to personnel, residents, visitors, and government agencies as required.
  • Documentation and Charting: Complete and file required recordkeeping forms/charts at admission, transfer, and discharge. Encourage physicians to review and sign orders and progress notes. Receive and transcribe physician orders to resident charts, cardex, medication cards, and care plans. Chart nurses’ notes descriptively, file accident/incident reports, and forward to the Director as required. Maintain Daily Census Report and submit to the Business Office as required.

    Ensure documentation compliance, including disclosure and privacy policies.
  • Drug Administration and Medication Management: Prepare and administer medications as ordered. Verify resident identity before administration. Ensure meds are not administered to the wrong resident. Maintain floor stock, order medications and supplies as needed, and ensure narcotic records are accurate. Notify the Nurse Supervisor of discrepancies and review medication cards for accuracy and stop-order adherence. Dispose of drugs per procedures and report any issues to the Nurse Supervisor.
  • Personnel Functions: Participate in staffing evaluations and scheduling; lead nursing personnel on your unit/shift; perform daily rounds to ensure standards are met; ensure trainees are supervised; enforce department policies; foster interdepartmental collaboration; create a positive unit atmosphere; address complaints and report issues to the Nurse Supervisor. Ensure CNAs are trained and enrolled, and that direct care is aligned with the resident’s care plan.
  • Nursing Care Functions: Inform and prepare rooms for new admissions; greet residents; orient new residents/families; supervise rounds with physicians; arrange diagnostic/therapeutic services; consult with physicians; implement nursing objectives and standards; monitor treatment administration by CNAs; coordinate social and activity programs; notify physicians and families of changes; carry out restorative programs; inspect treatment areas for cleanliness and safety; perform professional nursing procedures as required;
