We are seeking a Community Liaison to help educate, connect, and support patients, families, and healthcare professionals about hospice care. This is a dynamic role where your networking, relationship‑building, and communication skills will directly impact the lives of patients and families in need of compassionate care.

• Build and maintain relationships with healthcare providers, community partners, and referral sources.
• Educate the community on hospice benefits and services, ensuring accurate and up-to-date information is shared.
• Plan and implement marketing and outreach strategies to generate qualified referrals.
• Represent the organization with credibility, compassion, and professionalism at community events, health fairs, and networking opportunities.
• Collaborate with internal teams to ensure timely patient admissions and follow‑up.
• Maintain accurate records in CRM systems and report on community engagement efforts.

• Proven experience in relationship‑building, sales, or community outreach.
• Knowledge of hospice or healthcare services is preferred.
• Strong verbal and written communication skills; ability to engage with patients, families, and professionals.
• Self‑motivated, organized, and comfortable working independently.
• CPR/BLS certification and reliable transportation required.
• Flexibility to attend occasional evening or weekend events.
