Education and/or Experience
  • Master’s Social Worker in the jurisdiction(s) in which services are rendered.
  • 3 Plus years work experience in Care Management of HIV/AIDS required
  • Extensive knowledge of Ryan White Care Act as it relates to the MCM contracts as well as the overall Ryan White Care Act.
  • Five plus years of management experience preferred.

Your Contribution to Our Success:

A Licensed Clinical Care Manager contributes to AHF by utilizing their mental health expertise and understanding the Ryan White Program within their specific area. With a caring and knowledgeable focus, you will work collaboratively with the patients, our physician, and the Healthcare Center team to address the needs identified in the patient’s health and psychosocial Comprehensive Needs Assessment and implement an Individualized Care Plan based upon the needs identified, facilitate, and identify service options for addressing a client’s needs.

This will include providing education and assistance so that eligibility for the program is understood. For Ryan White contracts that combine psychosocial needs and resources within Medical Care Management, you will understand mental health conditions or substance abuse patterns that interfere with his/her ability to improve the client’s health and social outcomes.

For Ryan White contracts that combine psychosocial needs within Medical Care Management, you will understand mental health needs and basic needs such as housing, food, and clothing and develop relationships with programs and agencies serving those needs and facilitate referrals, support, and assistance for your patients who lack those services. You will also conduct ongoing retention calls to ensure no gap in retention efforts and to uphold radical cooperation across divisions.

A Licensed Clinical Care Manager enhances the reputation of AHF by utilizing your clinical expertise, you will screen clients for complex medical care and psychosocial issues requiring medical case management services. With your skills of collaboration and engagement, you will bring the essence of the Ryan White Care Act - the safety net for uninsured HIV/AIDS patients - to our most vulnerable population.

By establishing Comprehensive Assessments, Individualized Care Plans, follow-ups, referrals, and direction of clients through the entire spectrum of health and support services, your work will directly contribute to AHF’s success in meeting the deliverables of our Ryan White contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.

You will assure the coordination, follow-up, and completion of medical treatments and/or care plan interventions so that these services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care through ongoing assessment of the client’s and other key family members’ needs and personal support systems. You will verify the client’s eligibility for the ACA (Affordable Care Act) program and enroll those who qualify to ensure broader access to healthcare services.
