Since 2015, Nevada Behavioral Health Systems (NBH) has been providing mental health and substance abuse services to residents across Nevada. In 2023 we rebranded as Novum Health and expanded services to Arizona, California, and Utah, building a community‑based care system that offers personalized options tailored to individual needs.

Schedule
:
Thursday – Monday (7:30 am – 4:00 pm)
Location

Las Vegas, Nevada – Onsite (not eligible for Remote or Hybrid)
Starting Salary Range
: $23 per hour, subject to final offer based on experience, education, certification, and internal pay equity.

What’s the role?

Community Health Workers (CHWs) at Novum Health are trusted members of the community who help individuals navigate challenges with mental health, substance use, and social barriers. CHWs meet people where they are, whether at home, in the community, or in recovery settings, building trust, providing support, and connecting members with care and resources.

What you’ll do
  • Assess the healthcare needs of individuals within the community and identify gaps in care.
  • Collaborate with healthcare providers, social service agencies, and community organizations to develop comprehensive care plans for patients.
  • Assist patients in navigating the healthcare system and accessing necessary services, such as medical appointments, specialist referrals, diagnostic tests, and community resources.
  • Coordinate and schedule appointments for patients with multiple healthcare providers to ensure continuity of care and optimal care coordination.
  • Facilitate communication between patients, providers, and community partners.
  • Monitor patients’ progress and adherence to care plans, providing support and education to promote self‑management and prevention.
  • Conduct home visits or outreach activities to assess patients’ living conditions, social support systems, and adherence to treatment plans.
  • Advocate for patients’ needs and rights within the healthcare system and community.
  • Document patient interactions, care plans, and progress notes in electronic health record and care management systems.
  • Participate in interdisciplinary team meetings and case conferences to ensure effective coordination and collaboration with other healthcare professionals.
  • Provide transportation, with a personal vehicle, for members to appointments when needed (mileage is reimbursed).
  • Perform additional duties as assigned.
  • Build strong relationships with individuals and families to promote trust and reduce stigma.
What you’ll bring
  • Comprehensive knowledge of healthcare systems, community resources, and social services available to support patient care.
  • Knowledge of behavioral health, substance use treatment, and community resources.
  • Strong interpersonal and communication skills to establish rapport with patients, families, and healthcare providers.
  • Excellent organizational and time‑management skills to manage multiple patients and coordinate care effectively.
  • Ability to assess patient needs, develop individualized care plans, and evaluate outcomes.
  • Proficiency in using electronic health records and care management software.
  • Understanding of privacy regulations and compliance requirements related to patient information and data security.
  • Compassionate and patient‑centered approach to care, with a commitment to advocating for vulnerable populations.
  • Problem‑solving and critical thinking skills to address barriers to care and resolve complex situations.
  • Cultural competence and sensitivity to diverse populations and their unique healthcare needs.
  • Ability to work collaboratively in a team and maintain positive relationships with healthcare professionals and community partners.
What you’ll need to start
  • At least 1 year of experience in community health, behavioral health, peer support, or related roles.
  • Bachelor’s degree in healthcare, social services, or a related field (preferred).
  • An active and valid Community Health Worker Certification.
