• Collaborate with APP providers as a board-certified physician in Family Medicine:
  Sports Medicine Fellowship trained (or similar) for licensing and credentialing support as required by Nevada law.
• Maintain physical practice in Nevada as needed for licensing and credentialing.
• Move quickly to assist with updating CAQH profiles and credentialing submissions.

• Education:
  
  MD degree from an accredited medical school
• Licensure:
  Nevada
• Certifications:
  
  Board certification in Family Medicine:
  Sports Medicine, Physical Medicine & Rehabilitation, or similar

• Hours:
  
  1-2 per month
