Provides services to Medical Assisted Treatment. Assists in developing, evaluating, and coordinating treatment plans and recovery services.

• Demonstrate competency in managed care systems and provider networks in the adult recovery model.
• Demonstrate competency in case management procedures and resource allocation.
• Demonstrate awareness of social determinants of health.
• Adopt a culturally sensitive and non‑stigmatizing approach.
• Display familiarity with models of care management.
• Adopt a patient‑centered approach emphasizing self‑determination and shared decision making.
• Maintain confidentiality and discretion.
• Work effectively within an interdisciplinary team and communicate effectively.
• Familiar with all personnel and program policies and procedures.
• Multi‑task, prioritize, and work under pressure.
• Effectively use electronic health records.

• Bachelor’s degree in a human service related field or Associate’s degree plus two years of behavioral health or substance use treatment experience.
• Willingness to work unconventional hours (weekends, evenings, holidays).
• Valid New Mexico driver’s license, current automobile insurance, and a clean driving record.
• Willingness to use a personal vehicle for work when needed.
• Must submit to LCDF required background check, TB screen, and drug testing.
