Licensure:
You must have an active nm license  or reactivating one. Currently, obtaining a license takes 6 months if the applicant is new to practice; otherwise, it takes approximately 3 months.

** for coi's, providers must be enrolled in the nm patient compensation fund (pcf).

*** provider cannot be within 90 miles of the facility**
* The provider must contract with the agency before placing a bid.

Provider must be willing to work weekends, and take call.

Must be board-certified with at least one year of experience.

Acls, pals, and bls are required.

Must be comfortable practicing independently and performing regional blocks.

* The presentation must explain any gaps in service, malpractice claims, or sanctions from a board of nursing.
