The Home Health Field LPN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols.

Responsibilities include:

* Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse

* Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse

* Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status

* Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

Home Health Field LPN - Requirements:

* BLS (Basic Life Support)

* LPN (Registered Nurse) License

* Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

* Required Associate's or Diploma Nursing

* Required 1 year Licensed Practical Nurse

* Required1 year Home Care
