PT - Physical Therapist

This is a 13-week contract position.

We are seeking a dedicated Home Health Registered Nurse to provide skilled nursing care to patients in their homes. The ideal candidate will possess strong assessment and documentation skills, and be proficient in the use of medical equipment and devices.

• Conduct comprehensive patient assessments and create individualized home exercise programs.
• Administer medications and monitor patients for signs & symptoms of physical, sexual, or psychological abuse.
• Educate patients and their families on care plans and safety/injury prevention.
• Perform skilled nursing tasks, including but not limited to cardiac, ortho, neuro, and respiratory care.
• Ensure accurate documentation of patient care activities in compliance with OASIS standards.
• Manage 6-8 home health visits per day, adhering to scheduling and documentation requirements.

• Minimum of 1 year of home health experience.
• OASIS training preferred.
• Valid Florida state license or compact license.
• BLS certification required.
• Must be COVID vaccinated or have an approved exemption.
• Flu vaccination or declination required, with mandatory mask-wearing while at work.
