PositionSummary
This is a professional position providing Case Management services in the Family Works program under the Housing First Division. Family Works works with children and families to decrease homelessness and achieve stable, permanent housing. The role includes assessing, linking, coordinating, and monitoring services from mental health, social, and educational aspects. The team collaborates to develop service plans with families, working closely to identify needed services and establish a stable living environment.
Services are provided in‑home or in the community. The Housing Stability Case Manager plays an integral role in supporting, linking, coaching, and guiding youth and families who are homeless or housing‑unstable, often with mental health or co‑occurring substance‑abuse concerns. Responsibilities also include ensuring case management interventions are trauma‑informed, culturally and linguistically competent, and that the youth and family have an individualized treatment plan built collaboratively with the Integrated Decision Team (IDT).
The Case Manager reports progress, barriers, and safety issues during IDT meetings, ensuring client‑facing care hours of 1,410 annually.
- Must hold at least a Bachelor’s degree in Social Work or a related field from an accredited university; a Master’s degree is preferred.
- Experience with child welfare, early childhood education, or children’s mental health is preferred.
- Knowledge of community resources and child development (physical, mental, emotional, cultural) is preferred.
- Computer literate and proficient in Outlook, Excel, and Word; able to learn other software quickly.
- Must become a certified Wraparound Facilitator in high‑fidelity Wraparound within 3 months of employment; preference for existing certification.
- Must obtain and maintain BHCM certification.
