Position

Summary

This is a professional position providing Case Management services in the Family Works program under the Housing First Division. Family Works works with children and families to decrease homelessness and achieve stable, permanent housing. The role includes assessing, linking, coordinating, and monitoring services from mental health, social, and educational aspects. The team collaborates to develop service plans with families, working closely to identify needed services and establish a stable living environment.

The Housing Stability Case Manager plays an integral role in supporting, linking, coaching, and guiding youth and families who are homeless or housing‑unstable, often with mental health or co‑occurring substance‑abuse concerns. Responsibilities also include ensuring case management interventions are trauma‑informed, culturally and linguistically competent, and that the youth and family have an individualized treatment plan built collaboratively with the Integrated Decision Team (IDT).

The Case Manager reports progress, barriers, and safety issues during IDT meetings, ensuring client‑facing care hours of 1,410 annually.
