- Home Infusion Clinical Liaison, RN
Responsibilities include:
Facilitating transitions of care from acute and subacute settings to home infusion and education of home infusion and enteral therapies to hospital and community providers. - Assigned to selected facilities to collaborate with providers, case managers, referral sources, and home health agencies to facilitate patient discharge and coordination of care.
- Collaborate with the business development team in gaining and maintaining market share through the referral intake process.
- Maintain timely communication with referral sources telephonically, through electronic platforms, and in person. Ensure clear, concise referrals provided to pharmacy meeting all regulatory, payer, and safety requirements. Complete preadmission assessment and educate patient and caregiver of pharmacy services.
- Conduct the initial infusion and enteral setup and patient/caregiver training on our equipment and therapy services. Initiate formulary changes and recommendations on route of feeding and advances in nutrition under the direction of the pharmacy's registered dietitian.
- Maintain knowledge of Medicare and Managed Care regulations and requirements.
- Review the patient’s medical record to obtain pertinent medical history and primary/secondary insurance information and communicate this to pharmacy intake.
- Participate in data collection required for therapy start and patient tracking processes, including facilitating the transfer of orders via phone, fax, and e-prescribing.
- Provide timely responses to referral sources, providers, and leadership; monitor and communicate referral source activity as a single point of contact for referral sources, home health, and infusion.
- Will be responsible for additional transitions of care duties as assigned.
- BLS (Basic Life Support)
- Driver's License – State of Florida
- RN (Registered Nurse) License
- Associate's or Diploma Nursing (Required)
- 2 years of Nursing experience; 1 year of Infusion Related experience (Required)
- Business Development skills (Required)
- Reliable transportation
- Infusion and enteral administration skills assessment (Required)
- Computer proficiency in Microsoft Office and Excel (Preferred)
- Experience in Home Healthcare specialty Pharmacy (Preferred)
- CRNI Certification (Preferred)
