This position is responsible for safely and effectively transitioning members from acute or inpatient care to lower levels of care and/or home in a cost efficient manner. Provides assessment, planning, implementation, coordination, monitoring, and evaluation of services for members as they transition care and follows them for at least 30 days. Conducts an onsite hospital discharge visit and a post-discharge visit to assure continuity of care and prevent unnecessary re-admissions.

• Must have an active TX RN licensure
• Must have an active DL
• Must have at least 3 years of clinical experience, this is a must!
• Minimum two years Medical Case Management experience or Public Health experience.
• Knowledge or experience using the Care Transitions Intervention or similar model
• Background in discharge planning and or home health or managed care
