Maternal Child Health Home Visiting Nurse

Albany County Public Health is seeking a full-time (40 hours/week) Maternal Child Health Home Visiting Nurse to join our supportive and collaborative team. This position offers a family-friendly schedule that promotes flexibility, professional autonomy, and a strong work–life balance.

This role supports the implementation of the public health Maternal Early Childhood Sustained Home-Visiting (MECSH) program, Wyoming Hand in Hand, delivering evidence-based home visiting services to improve maternal and child health outcomes.

Key Responsibilities
  • Deliver evidence-based maternal and child health home visiting services to pregnant women and families through the MECSH program, Wyoming Hand in Hand
  • Serve in a field-based nursing role, spending approximately 50–75% of work time in home and community settings
  • Build trusted, supportive relationships with families to promote healthy pregnancies, child development, and positive parenting practices
  • Independently schedule and conduct home visits, supporting flexibility and autonomy in managing daily workloads
  • Complete health assessments, screenings, education, and referrals tailored to individual family needs
  • Participate in outreach activities, immunization clinics, and Public Health Emergency Preparedness efforts as needed
  • Collaborate with a multidisciplinary public health team and community partners to ensure comprehensive, coordinated family support
  • Maintain accurate and timely documentation and reporting in accordance with program and public health standards
  • Participate in mandatory training, which may occasionally require overnight travel, as well as ongoing continuing education and program development
  • Uphold confidentiality, ethical standards, and culturally responsive care in all interactions
Qualifications
  • Wyoming Registered Nurse (RN) license, or ability to obtain licensure within 30 days of hire (required)
  • Bachelor of Science in Nursing (BSN) preferred
  • Experience in pediatric, maternal/child health, home visiting, and/or public health preferred
  • Strong computer and electronic documentation skills
  • Ability to work independently with strong communication and interpersonal skills when collaborating with families, coworkers, and community partners
  • Demonstrated ability to build trusting relationships, exercise sound judgment, and apply effective problem-solving skills
  • BLS/CPR for Healthcare Professionals certification (required)
  • Valid driver’s license with a satisfactory driving record

Special Requirements
  • Ability to lift, transfer, and reposition clients as needed
  • Ability to lift and transport equipment weighing up to 50 pounds, including to off-site clinics and classes
  • Ability to restrain clients for treatment, climb stairs to patient homes, and drive extended distances
  • Willingness to travel within the state and attend out-of-state or national conferences as requested
  • Ability to work occasional evenings, nights, and/or weekends when public health needs require it (typically related to special events, training, or public health response)
Conditions of Employment

As a condition of employment, the successful applicant will be required to complete an Employee Health Assessment, which includes verification of immunity to vaccine-preventable diseases. Applicants found to be non-immune will be required to receive appropriate vaccinations unless medically contraindicated. Annual influenza vaccination is also recommended unless medically contraindicated.

A background check, including fingerprinting, will be completed for all successful applicants through the Wyoming Department of Criminal Investigation, the Department of Family Services, and the Federal Bureau of Investigation.
