Leading Home Care, the leading Home and Community-Based Services (HCBS) provider for Medicaid waiver services in Louisiana, is expanding its state-wide footprint and opening a new office in LaPlace, LA. We are looking for an eager, talented and compassionate
Client Care Manager
to lead this new team.

NOTE:This position will require daily commute to one of our established regional offices for training during the first 2 months of employment (Thibodaux, Hammond, or Baton Rouge). Following training, the Client Care Manager will work out of the new LaPlace office. Commute time & mileage will be paid.

HCBS companies provide opportunities for Medicaid beneficiaries to receive home care services in their home rather than in institutions or nursing homes. These programs serve a variety of population groups including elderly, Veterans with VA benefits, and people with intellectual or developmental disabilities, physical disabilities or other illnesses.

Client Care Manager

Responsibilities:
  • Manage care for clients who receive in-home personal care services
  • Create & update person-centered Care Plans
  • Ensure care plans are carried out by Caregivers and that clients' health and well-being are maintained
  • Supervise, train, evaluate, and provide direction to Caregivers in regards to all aspects of client care
  • Communicate frequently with clients, families, and other affiliates to answer questions, resolve issues, and ensure satisfaction with services
  • Travel to clients' homes for meetings and visits quarterly, annually, and as needed.
  • Properly document all client communications, care plan or medical changes, visit notes, or incidents.
  • Maintain digital & paper records for each client
  • Coordinate with Director, Staffing Coordinator, and HR on a regular basis to discuss any needs or issues regarding clients or employees
  • Participate in after hours on-call rotation and serve as backup staff working in-home with clients as needed
  • Comply with all program guidelines and meet requirements established by LDH, Medicaid, and LHCL.
  • Promote LHCL in the local community and build relationships with external stakeholders
  • Model the LHCL Mission, Values, & Leadership Behaviors
  • Additional tasks related to Client Care as needed
Requirements:
  • Bachelor's degree required, preferably in Human Services or related field
  • 4 years' experience preferred (home care industry, case management, working with elderly/disabled, or other related experience)
  • Strong management, communication and organization skills
  • Must satisfactorily pass all required and applicable State & Federal eligibility verifications including background, driver's license, and other required screenings
  • Basic computer skills including Microsoft Office, Word, and Excel
  • Able to operate office equipment
  • Basic addition/subtraction and general analytical skills
  • Valid driver's license, auto insurance, and reliable transportation
  • Must be able to read, write, and follow instructions in English
  • Physical requirements include stooping, kneeling, reaching, standing, walking, and climbing stairs. Must be able to occasionally lift up to 50lbs (carrying files/boxes, assisting with client lifts/transfers, etc.)

Office hours are 8:30 a.m.

- 4:30 p.m. Monday through Thursday and 8:30 a.m.

- 2:00 p.m. Friday.
