Pay: $42.00-$47.50 per hour
The Clinic RN, under the direction and supervision of the Clinic Supervisor, provides skilled nursing care to participants, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plans of care, provides participant, caregiver and family teaching, and communicates participants’ condition to primary caregivers, physicians, and the Interdisciplinary Team.
Required Qualifications- Graduate of a school of professional nursing
- Current state Registered Nurses (R.N.) License
- Cardiopulmonary Resuscitation (CPR) certification required
- A minimum of one year’s experience working with the frail or elderly population
- Valid driver’s license, proof of insurance, and means of transportation
- Medical clearance for communicable diseases and up-to-date immunizations before direct participant contact
- Bachelor of Science Nursing (BSN) preferred
- Conduct initial comprehensive nursing assessment with new PACE enrollees and coordinate with the Interdisciplinary Team to develop comprehensive plans of care.
- Conduct in-person nursing re-assessment semi-annually and as needed.
- Act as a member of the Interdisciplinary Team; attend meetings and communicate participant changes, collaborate on plan of care decisions, and coordinate 24‑hour care delivery.
- Provide orientation, initial and annual competencies for Licensed Practical Nurse (LPN), Certified Nursing Assistant (CNA), and Clinic Clerk.
- Perform competencies on appropriate day center staff as needed.
- Perform general nursing care; supervise administration of prescribed medications and treatments in accordance with nursing standards.
- Triage participants to determine priority of care.
