The Hospice After Hours Registered Nurse (RN) provides essential nursing care for terminally ill Hospice patients during evening, overnight and weekend shifts. Responsibilities include supporting patients and their families in home care settings, offering guidance and empathy during difficult times, and providing quality care to our patients within their home.
Required Experience- Graduate from an accredited school of nursing
- Currently licensed as hospice registered nurse to practice in MI
- Minimum 2 years of experience as a registered nurse (preferred)
- Active patient contact within past 3 years (preferred)
- Ability to work independently, make accurate and quick judgments
- Ability to supervise others appropriately
- Ability to respond appropriately to crisis outside of a hospital setting
- Acceptance of and adaptability to different social, racial, cultural and religious modes
- Regularly assess and reassess the nursing needs of the Hospice patients in the patient’s home as needed.
- Provides dietary counseling.
- Provides Hospice nursing services, treatments and preventive procedures.
- Initiates nursing procedures appropriate for the patient’s Hospice care and safety.
- Observe signs and symptoms and report to the physician and IDG members of any unexpected changes in the patient’s physical or emotional condition.
- Teach, supervise, and counsel the Hospice patient and family members about providing care for the patient.
- Supervise and train other nursing service personnel.
- Develop and re-evaluate the patient/family care plan with IDG to meet needs and maintain continuity of care.
- Perform specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor’s orders.
- Keep abreast of business updates by actively participating in company meetings or reviewing recordings.
- Maintain records as required by Hospice.
- Follow hospice policies and procedures; observe confidentiality and safeguard all patient-related information in compliance with HIPAA.
- Always communicate to the On-Call Clinical Manager if unable to meet a patient’s need or perform a procedure.
- After-hours nurses will be required to provide coverage as needed, demonstrating flexibility and readiness to support patients during critical times.
- Work collaboratively with the After Hours Team to ensure patient needs are met.
- Work with the interdisciplinary group concept of patient care.
- Coordinate the implementation of the plan of care for patients residing in SNF, NF, ICF or MR.
- Organize work schedule and use time management to attend all required meetings.
- Comply with agency infection control policies and protocols.
- Assist with orientation, teaching and training as requested.
- Other duties as assigned by Director of Clinical Services or Clinical Manager.
