Job Details

Pay: $18.00 - $23.00

Shift: Operating hours between 7:00 AM – 9:00 PM; 8 hours per shift; 1 weekend per month.

Responsibilities
  • Under the direction and supervision of the Home Care Manager and Personal Care Supervisor, provide and assist participants with activities of daily living, restorative and supportive care to help participants live independently in the community.
  • Monitor and report health status and maintain a safe environment for PACE participants.
  • Functions as a member of the Interdisciplinary Team.
  • Consult daily with the Personal Care Supervisor (or designee) to be updated on participant status and changes in plan of care; confirm scheduled visits.
  • Assist participants in the home and day center with duties including personal hygiene, ambulation, prescribed exercises, meal preparation and feeding, bathroom assistance, and medication assistance as needed.
  • Assist participants with Independent Activities of Daily Living (IADL) such as grocery shopping, cleaning, changing bed linens, laundry, meal preparation, and other routine household tasks; report changes to the supervisor.
  • Complete appropriate documentation of services provided following policy; follow all Senior Community Care policies and OSHA safety guidelines.
  • Maintain confidentiality of all company procedures and participant information.
Required Qualifications
  • Current License as Certified Nursing Assistant (C.N.A) in the state of work
  • High School diploma or equivalent
  • Minimum of one year’s experience with frail or elderly populations
  • Valid driver’s license, proof of insurance, and reliable transportation
  • Medical clearance for communicable diseases and up-to-date immunizations before direct participant contact
Essentials
  • Consult daily with the Personal Care Supervisor to be updated on participant status, plan of care changes, and scheduled visits
  • Assist participants in the home and day center with duties including personal hygiene, ambulation, prescribed exercises, meal prep and feeding, toileting, and assisting with medications as needed
  • Assist with IADLs such as shopping, cleaning, laundry, and meal preparation; report changes to supervisor
  • Complete documentation of services provided following Senior Community Care policy
  • Adhere to all company policies and OSHA safety guidelines
  • Maintain confidentiality of participant information
