In this role, you will be responsible for providing medically related social services to support the well‑being of our residents and their families. Participate in the admission process in collaboration with the Nursing Home Administrator and Director of Nursing. Assess residents’ psychosocial needs and deliver individualized support services, as needed.

• Bachelor’s degree in social services or a related field
• Minimum of two years’ experience in a long‑term care setting
• General knowledge of social services required in a healthcare setting
